The application asks only for business contact and clinic-operational information. Form submissions are processed and stored by our website hosting provider so we can review and respond to your inquiry.

Information you must not submit

Do not submit patient names, symptoms, diagnoses, prescriptions, laboratory results, medical records, insurance information, or other patient or protected health information through this website or email.

Service providers

Form submissions are processed by Netlify, and booking links open Calendly. Those providers process information under their own privacy terms. We use submitted business information only to evaluate and respond to your inquiry.

Clinic responsibility

Clinics remain responsible for legal, privacy, clinical, and regulatory review, including appropriate security controls, access limits, vendor review, and business-associate arrangements where required.
